Terms and Conditions of Service

1. All clients of Doncaster Veterinary Hospital must be kind and courteous to staff at all times.

2. Service will be refused if clients are rude or abusive to any staff members of Doncaster Veterinary Hospital.

3. Payment in full is required at the time of service.

4. Failure to provide payment in full will result in termination of future service with Doncaster Veterinary Hospital unless payment is made in full.

5. For clients with stored reproductive material (semen), the semen will be destroyed after 7 days of the client owing money for any reason.

6. Doncaster Veterinary Hospital has the right to terminate any previous agreement of service and refuse to provide any future service to clients which breach these conditions of service.

7. Patient records are the property of Doncaster Veterinary Hospital and will only be provided to other veterinarians in the form of a full clinical history from the records of Doncaster Veterinary Hospital, or a clinical summary.  The form of how the history/records are provided is up to the discretion of Doncaster Veterinary Hospital.  The detail of such records will be determined by Doncaster Veterinary Hospital to allow continued treatment of the animal in question by future veterinarians as per Veterinary Practitioners Registration Board of Victoria recommendations.

8. No clinical histories of, or summaries of, any breeding management including breeding by natural mating, or artificial insemination of fresh, chilled or frozen semen will be provided to other veterinarians or their owners. Information of the calculated whelping/queening (or due) date will be provided at the end of the breeding management process to the client of the animal to allow for continued care of the animal by other veterinarians.

9. Pregnancy ultrasound dates on the information provided to the client at the end of breeding management is calculated at a date that Doncaster Veterinary Hospital is able to determine pregnancy. Doncaster Veterinary Hospital takes no responsibility in the minimum gestational age able to be detected by other veterinary practices as this may differ from Doncaster Veterinary Hospital.
